In a nutshell, the problem was cartridge construction and metal choice. Choice is perhaps an unfair word, because deep-cup metal drawing was in its' infancy, and a soft alloy was required. The "copper" (which expands but does not contract) cases, with soft folded heads, were prone to stick in the chamber. Dirt further aggravated the condition. The change to a more sturdy head (with the Boxer primer) and later to "brass" cases, even later to "solid-head" completely solved the problem. As you say, the TD has a very positive ejection system - given the proper item to hurl.
